Patriot Insurance Company is no longer issuing new life insurance policies through Patriot Life Insurance Company. However, if you have a policy currently in force, we remain committed to honoring the terms throughout the life of your policy.
For instructions on how to get information for your life insurance policy, please read below.
Reporting a life insurance claim.
To report a claim with Patriot Life Insurance Company, please contact us by phone at 844-847-0793.
Please be prepared to have the following information ready:
- Your policy number, which will begin with “LIF” followed by a seven-digit number.
- The issue date of the policy.
- The date of death.
- Beneficiary name(s) and contact information (including phone number, email address, and mailing address).
Claim documents and instructions will be sent directly to any named beneficiaries for additional information needed.
To process the claim for payment, please provide an original death certificate with the state seal on it.
Each beneficiary will need to provide their own notarized/witnessed copy of the completed claim form and an IRS W-9 Form.
If the claim is within the two-year contestability term, additional information may be required.
