To improve our overall efficiency Patriot Insurance has contracted an outside vendor for electronic notification of Additional Interest notices.  As part of this agreement we will no longer be mailing certain paper copies to Additional Interests. 

Direct Bill Policies

The following forms for Direct Bill policies will now be sent electronically to Additional Interests:

·         HP Evidence of Insurance form 96226 

·         PA Evidence of Insurance forms FM-821 (IO), FM-822 (LP), FM-825 (AI)

·         HP & PA Removal Notices form 94158 

 Please note there are some exceptions which will continue to print, archive, and be mailed including:

·         Loss Payees on Homeowners (Evidences of Insurance only – Removal Notices will be sent electronically)

·         Joint-Owner forms on Personal Auto (Evidences of Insurance only – Removal Notices will be sent electronically).

Also, please be aware that these forms will no longer be available for you to view from the Associated Documents option in Policy Inqury.
