Frankenmuth Insurance has revised its Employment Practices Liability Insurance (EPLI) application.  Updates and editorial changes designed to make the application process more efficient are now in place.

For your convenience, be sure to use this new EPLI application with the edition date of 08-14 for all of your new and renewal business effective 8-1-2014 or later.  A link to the new FM-1113 form can be accessed below.

Should you have EPLI applications with edition dates 01-13 or earlier, please disregard them.  After our current effective date of 8-1-2014, those previous applications will not be accepted.
