I have reason to believe someone may be committing insurance fraud. What should I do?
If you think someone may be committing insurance fraud, please call our fraud line at 866-460-1776 or email us at patriotclaims@fmins.com so we can put a stop to it. You can also submit an anonymous tip online.
How do I submit information for my claim?
Claim documents can be submitted via email to PatriotClaims@fmins.com or by fax to 888-235-7289. When submitting documents, include your claim number or policy number in the subject line or on a cover sheet.
If I’m served with a lawsuit, what should I do?
Let us know by filing a claim immediately. You will need to submit a copy of the lawsuit and provide details such as the date you were served and the method of service (e.g., in person, by mail). We’ll review the lawsuit and your insurance policy, and then we’ll investigate to see if you’re covered. If your insurance policy includes it, we will defend you, and we will cover the cost of your defense. You do, however, have a right to hire your own attorney to defend you for damages that may be in excess of your policy limit or are not covered in your insurance policy.
If I’m in a car accident and my vehicle is damaged, do I need to get two estimates?
Generally, only one estimate is needed. Some vehicles may be eligible for photo-based estimates. Your adjuster will go over the best option for your situation.
If I’m in a car accident and my vehicle is taken to a tow yard, what do I do?
Does my deductible apply if my windshield is repaired from a stone chip?
Generally, no. The deductible on an auto claim only applies if the entire windshield is replaced. Please contact your agent with any questions. Before having any glass repaired or replaced, report an auto glass claim online, or call 866-460-1776 to report by phone.
If I hit a large pothole in the road and damage the underside of my car, is that considered a collision?
Yes. Our auto insurance defines collision as the upset of your vehicle or its impact with another vehicle or object. In this case, the road surface would be considered an object.
How do I file a claim for towing and labor reimbursement?
Either contact your agent to help you file the claim or file the claim yourself. Make sure you have the following information available:
- Valid policyholder email address
- Valid policyholder mobile phone number
- Amount of towing and labor service
- A copy of the towing and labor service receipt
You will receive two emails as part of the automatic claim process. One email will provide you with your claim number, and the second email will allow you to select your preferred method of payment for the automatically processed claim.
An automatic payment will be processed to the primary named insured only. The primary insured’s name can be checked by looking at the name displayed in the “Draft Made Payable” field.
If I rent a car and it’s damaged, what do I do?
If my home, business, or vehicle is vandalized or my personal property or vehicle is stolen, what should I do?
Please notify the police and contact us by filing a claim immediately.
If my home or business is damaged, what should I do?
Notify us immediately by filing a claim, and then take all reasonable measures to protect your property from further damage, as required by your policy.
If a tree falls on my home, does my homeowners policy provide any coverage?
Yes. We will pay the reasonable cost to remove the tree from your home. Your policy also provides coverage for repairs to your home and limited coverage to remove the fallen trees from the premises. Please notify us by filing a home insurance claim immediately.
Does my homeowners deductible apply to every claim?
Yes. The deductible applies on a per-claim basis. If multiple claims occur in a year, a deductible applies each time a separate claim takes place.
Does my home insurance policy cover flood damage?
Typically, home insurance policies do not cover damage caused by flood or surface water. However, you may be able to purchase additional coverage. Talk to your local, independent agent today.
If wind blows down trees in my yard but they don’t hit my house, do I have coverage to remove the trees?
No. We do not cover the cost of removing trees that have blown down in your yard unless the tree hits your home or a covered structure. If the tree does hit your home or a covered structure, we will pay up to $500 to remove the fallen tree from the premises.
What is "scheduled property?"
“Scheduled property” is property that is specifically described and insured on an endorsement that’s been added to your insurance policy. This endorsement has its own agreement and conditions, which require us to replace the lost or damaged property with property of like kind and quality, or pay you up to the specific limit for the item as stated in the policy.
Why do you have to put the bank’s name on my insurance claim payment?
We are required to protect the interests of the entity that holds a lien or mortgage on your property. If we fail to protect their interest and the property is not repaired after a loss, we may be held liable.
How do I report a workers compensation claim if I’m injured at work?
Please ask your employer to complete a report of injury form and notify us by filing a claim within 24 hours of the injury. In partnership with Medcor, Frankenmuth Insurance offers an injury triage service for workers compensation policyholders. The service gives supervisors and injured employees immediate access to trained healthcare professionals at no additional cost. If the injury is potentially life-threatening, call 911 immediately. For non-life-threatening injuries, your supervisor can call 833-833-1934 for triage service. If a supervisor is not available, an injured employee can call directly. If I receive workers compensation benefits, will I receive a W-2 form?
No. Workers compensation benefits are nontaxable.
How do I file a claim for a life policy?
To file a claim for a life policy, please complete the following steps.
Contact Patriot Life Insurance Company at 844-847-0793 and provide the below information:
- Your policy number, which begins with “LIF” followed by a seven-digit number.
- Issue date of policy.
- Date of death.
- Name, phone number, email address, and mailing address for every named beneficiary.
Claim documents and instructions for additional information will be mailed to the beneficiary/beneficiaries You can contact your independent agent to help you through this process. The below documents will need to be sent to Patriot Life Insurance Company:
- A certified copy of the death certificate that includes the state seal.
- Notarized/witnessed copies of the completed claim document and IRS W-9 forms for each beneficiary.
- Additional information may be required if the claim is within the two-year contestability term.
